How It Works
Semaglutide is an acylated GLP-1 analog with 94% homology to native GLP-1 and an extended half-life of approximately 7 days due to albumin binding. It activates GLP-1 receptors in the pancreas to enhance glucose-dependent insulin secretion, suppresses glucagon, and slows gastric emptying. Centrally, it acts on hypothalamic appetite centers to reduce food intake, producing sustained weight loss of 15-17% of body weight in clinical trials.

Common side effects include nausea, vomiting, diarrhea, and constipation, which typically decrease over time. Carries a boxed warning for thyroid C-cell tumors (observed in rodents). Contraindicated in patients with personal or family history of medullary thyroid carcinoma or MEN2 syndrome. Risk of pancreatitis, gallbladder disease, and hypoglycemia (especially with sulfonylureas or insulin). Requires dose escalation to reduce GI side effects.
