Methylene Blue
A century-old redox-active dye repurposed as a mitochondrial enhancer, neuroprotectant, and alternative electron carrier.
Also known as: Methylthioninium chloride, MB, Provayblue

Compounding: Legal through licensed 503A/503B pharmacies
Evidence Summary
How It Works
Methylene blue acts as an alternative electron carrier in the mitochondrial electron transport chain, shuttling electrons directly from NADH to cytochrome c, bypassing dysfunctional complex I and III. This increases ATP production and reduces reactive oxygen species leakage. At low doses it enhances mitochondrial respiration and has demonstrated neuroprotective effects in models of Alzheimer's disease and traumatic brain injury, partly through inhibition of tau aggregation and monoamine oxidase.
Common Applications
Safety & Considerations
Safe at low doses (0.5-2 mg/kg); higher doses paradoxically increase oxidative stress. Causes blue-green discoloration of urine and may temporarily tint skin. Contraindicated with serotonergic medications (SSRIs, SNRIs, MAOIs) due to risk of serotonin syndrome. Not recommended in G6PD deficiency due to risk of hemolytic anemia. Should be pharmaceutical grade only.
